JOB DESCRIPTION:

GENERAL SUMMARY:

Under the direct supervision of the nurse, offers the care, safety, convenience and personal privacy of patients based upon their individual requirements. Might carry out responsibilities in a range of settings consisting of the patients place of residence.

ESSENTIAL DUTIES:

• Supplies, gets or carries out and documents direct client care and activities such as: activities of daily living, skin study, essential indications, ambulation, specimen collection, consumption and output, meal usage, height and weight, blood glucose testing/monitoring, point of care testing, bladder scanning, moving of patients and their charts, paperwork of O2, assistance with tests, treatments and procedures. May also total 12 lead EKG testing. This work might involve offering services at the patients place of residence. (30%)
• Takes part in quality and patient satisfaction initiatives which might include per hour rounding promoting safety such as fall prevention, hand washing, isolation procedure and preserving and orderly environment. (30%)
• Reports modifications in client’s physical, mental and psychological conditions, as well as any problems or concerns recognized to nursing personnel. (20%)

QUALIFICATIONS:

Minimum

• High school diploma or GED; or one to three months related experience and/or training; or comparable combination of education and experience.
• CPR accreditation.
• For House Health work, proper state licensure.

Preferred

• CNA certification.
• Previous Nursing Assistant experience.
